How to Apply:

1. Application forms are available in Academic Section, MTVMMC, Mardan.
2. Attested copies of the following documents should be attached with the application form and original documents must be shown at the time of interview:
a. MBBS Degree.
b. MBBS Academic Certificate.
c. Character Certificate
d. F.Sc Certificate.
e. Matric Certificate.
f. PMDC Registration
g  N.I.C
h. Domicile.
i. 03 pictures (passport size)
j. Undertaking on Judicial Stamp Paper (v... is &am* in Aadenvc section)

3. Application form must be filled in properly.
